About the Practice
We're a hospital-based practice — not a clinic or a private office. We care for medically complex patients across three settings: a long-term acute care (LTAC) hospital (our primary site and training ground), acute rehab units, and hospital backup units within area skilled nursing facilities — a specialized program our supervising physician helped pioneer for patients who are too medically complex for a standard nursing home but no longer need ICU-level care.
Our patients are often ventilator-dependent, medically fragile, and staying with us anywhere from a few weeks to a year, which means real relationships with patients and their families, not just quick visits.

We're a small, tight-knit, family-run practice and it shows: several of our NPs have been with us for years, some for a decade or more. We welcome new graduates — plenty of our best long-term team members started here straight out of school, and we invest heavily in training everyone, regardless of experience level, the specific way we practice medicine.

Position Overview
You'll provide hospital-based medical care to a highly complex, high-acuity adult population — patients can range from med-surg-stable to critically ill on the same hallway, so the ability to shift gears constantly is essential. Every new team member, regardless of prior experience, rounds directly with our supervising physician for a minimum of 3 months before rounding independently — this is a genuinely mentorship-first practice, not a sink-or-swim one. Twice a week, the whole care team does structured multidisciplinary rounds together (medicine, nutrition, and others at the table).
Key Responsibilities

Manage a caseload of medically complex, often ventilator-dependent patients across our LTAC and hospital backup unit sites.
Round independently after your mentorship period, while continuing to loop in physicians and senior NPs on anything outside your comfort zone.
Communicate extensively with patients' families — many of our patients can't advocate for themselves, so family communication is a core part of the job, not an afterthought.
Participate in twice-weekly team rounds and respond to on-the-floor questions from nursing staff even outside your own assigned patients (this is a "whole unit is your responsibility" culture, not a "just my patients" one).
Take phone call coverage as part of the on-call rotation (no in-person call required).
Document thoroughly and consistently with the practice's established charting approach.
Travel between facility sites as needed (compensated).

Required Qualifications

Licensed (or in the process of obtaining licensure by start date) as a Family, Acute Care, or Adult-Gerontology Nurse Practitioner, or as a Physician Assistant, in Colorado.
Genuinely coachable and willing to learn this practice's specific way of doing things, even if you're coming in with significant outside experience — this matters more than your resume. New graduates are welcome and encouraged to apply.
Willing to commit to a minimum 3-month physician-supervised rounding period before practicing independently, and ideally to a 2–3 year commitment given the training investment involved.
Comfortable working as part of a highly diverse team and serving a highly diverse patient population (varied ages, backgrounds, identities, and circumstances, including patients from correctional facilities) with respect and without judgment.
Prior RN or clinical experience in acute/critical care or hospital settings is a plus, evaluated case-by-case toward experience requirements — not a hard prerequisite.
DEA registration is not required to start if you are a new grad
- Experienced NPs would be required to have a DEA license

Clinical Opportunities
Every new hire gets structured, hands-on mentorship — you'll round with our supervising physician and senior NPs for at least 3 months, building toward independent practice. All continuing education (including a full week of paid CME time, airfare, program costs, and a meal per diem) and all licensing, CPR, and ACLS costs are covered by the practice. As our hospital backup unit program continues to grow, there's real room to grow with it for the right person.

Schedule and Work Environment

Full-time, fully onsite across our Denver-metro sites
12-hour shifts, roughly 14 shifts per month, including one full weekend per month; schedule varies week to week.
On-call is phone-only — no in-person call shifts.
Genuinely flexible around personal/family schedules where possible (this practice actively coordinates shift schedules around things like a partner's job or childcare needs).
Patient load ramps gradually as you gain experience, building toward roughly 24 patients/day.

Compensation and Benefits

Salary, with raises often reviewed at 3 months, 6 months, 1 year, and annually thereafter; experienced hires are brought up toward their prior earnings quickly if they're performing well.
Health and dental insurance provided (vision isn't currently offered but the practice is open to adding it).
15 PTO days in year one; no separate sick-day accrual — if you're out sick, you're covered without losing PTO (a doctor's note is needed beyond 2 days).
One week of fully paid CME time annually, including airfare, program cost, and a daily meal per diem
All licensing, CPR, and ACLS certification costs covered
