Role and responsibilities

This Nurse Practitioner (NP) is actively involved in coordinating the care of a patient receiving physical therapy within a Skilled Nursing Facility (SNF), ensuring all aspects of their treatment plan, including therapy sessions, medication management, and overall health status, are aligned and effectively communicated between different healthcare providers involved.

The Nurse Practitioner acts as a central point of contact, overseeing the patient's care plan and coordinating communication between the physical therapist, other healthcare professionals in the SNF (like nursing staff), and potentially the patient's primary care provider. 

  • SNF care: A Skilled Nursing Facility provides skilled nursing care, including physical therapy, to patients who need ongoing medical monitoring and assistance with daily activities after a hospital stay. 

  • Home Health Physical therapy: This is a key component of SNF care, focusing on restoring physical function through exercises and modalities to help patients regain mobility and independence. 

  • Care coordination: The NP ensures all aspects of the patient's care are aligned, including therapy schedules, medication management, progress updates, and communication with family members.

Key Tasks include:

  • Assessing patient needs: Conducting comprehensive assessments to identify the patient's functional limitations and therapy goals. 

  • Developing care plans: Collaborating with the physical therapist to create a personalized therapy plan based on the patient's needs and goals. 

  • Monitoring progress: Regularly reviewing the patient's progress with the physical therapist and making adjustments to the care plan as needed. 

  • Medication management: Ensuring appropriate medication administration and addressing potential medication side effects that could impact therapy. 


Communication with other healthcare providers: Keeping the primary care physician informed about the patient's progress in the SNF and coordinating any necessary follow-up care
