Job Responsibilities:

• Conduct virtual and some in-person consultations with patients to assess their health needs, particularly those with chronic conditions, and provide appropriate medical care.

• Develop and implement individualized care plans to improve patients' overall health and lifestyle, with a focus on managing chronic conditions.

• Analyze patient data from various sources, including wearable devices and health records, to inform treatment decisions and track progress.

• Monitor patient progress and adjust care plans as necessary based on data analysis and patient feedback.

• Collaborate with a multidisciplinary team, including lifestyle educators and data scientists, to provide holistic care.

• Utilize telehealth technology to maintain accurate and timely patient records.

• Educate patients on wellness, preventive care, and lifestyle changes to manage chronic conditions effectively.

• Stay current with medical research and best practices in telehealth, chronic care management, and lifestyle medicine.

Qualifications:

• Current and unrestricted NP or PA license.

• Experience in telehealth or willingness to learn telehealth platforms.

• Strong background in chronic care management and lifestyle medicine.

• Proficiency in data analysis and interpretation, particularly health-related data.

• Excellent communication and interpersonal skills.

• Ability to work independently and as part of a collaborative team.

• Commitment to providing compassionate, patient-centered care.

• Advanced technical skills to navigate telehealth systems, electronic medical records, and data analysis tools.

Preferred Qualifications:

• Certification in Chronic Care Management or related field.

• Experience with health data analytics and wearable health technology.

• Background in developing lifestyle interventions for chronic conditions.

• Familiarity with motivational interviewing and behavior change techniques.
