Job Requirements (NP / PA-C)
Master’s degree from an accredited Nurse Practitioner (NP) or Physician Assistant (PA) program
Active California state licensure as a Nurse Practitioner (NP) or Physician Assistant (PA-C)
Certification: NCCPA (for PA-C) or Acute Care NP certification preferred
Valid DEA license
Must reside in California (state-based licensure and telemedicine regulations)
Minimum of 5 years of clinical experience in internal medicine, emergency medicine, or as a hospitalist
Experience with telemedicine / virtual patient care preferred (hospitalist telehealth experience a plus)
Proficiency with electronic health records (EHR), especially EPIC
Strong clinical assessment, communication, and coordination skills for virtual care
Key Responsibilities
Perform comprehensive virtual assessments to evaluate patients’ medical conditions, history, and current symptoms
Review and interpret medical records, labs, and imaging to guide clinical decisions
Determine patient eligibility for the Hospital at Home program using established clinical criteria
Develop and implement individualized care plans in collaboration with a multidisciplinary hospitalist and home health team
Monitor and manage patients remotely, coordinating timely in-home services and follow-up with home health providers
Provide ongoing virtual follow-up assessments and adjust care plans based on clinical status
Communicate clearly with patients, families, and caregivers to educate and ensure understanding of at-home care plans
Coordinate care transitions with primary care physicians and specialists to maintain continuity of care
Document visits and maintain accurate, up-to-date patient records in the EHR in compliance with regulatory requirements

What You’ll Do
As an NP with IKC, you’ll serve as a key provider for high-risk patients in either CKD or ESKD stages. Working closely with nephrologists, dialysis teams, and interdisciplinary staff, you will:
Provide primary care within dialysis units or nephrologist offices
Manage chronic illness and co-morbid conditions such as heart failure (HF), hypertension (HTN), and diabetes (DM)
Reduce hospitalizations and total cost of care through preventive, patient-centered approaches
Help establish and grow new outpatient CKD clinics near dialysis units
Champion care redesign in an innovative, patient-first environment

Active North Carolina Nurse Practitioner license and Board Certification (NP Boards required)
BLS certification required
Experience treating both adult and pediatric patients (ages 6 months+)
Epic EMR proficiency or willingness to complete training
Strong communication and interpersonal skills
Ability to work independently as the sole healthcare provider
Healthcare Responsibilities
Deliver comprehensive urgent care to patients of all ages, including assessment, diagnosis, and treatment
Perform minor procedures: I&D, laceration repair, ear irrigation, and other urgent care interventions
Manage a high-volume patient load with compassion and efficiency
Accurately document patient encounters in Epic EMR
Collaborate with clinical support staff: Center Manager, Medical Assistant, Patient Service Associate
Participate in onboarding and ongoing clinical training sessions
