Job Requirements
Minimum 1 year of APP experience in Urgent Care and/or Occupational Health
Board Certified — Required
Active APP license in Virginia — Required
Current BLS certification from AHA — Required
Provider must be local to the Lynchburg, VA area or able to commute reliably
Ability to work autonomously with supervising physician available by phone
Self-Query NPDB report (pulled within 30 days) required at time of presentation
Responsibilities
Provide episodic acute care and injury treatment to adolescent, child, adult, and geriatric patients in an urgent care setting
Deliver occupational health services including work-related injury assessment, workers' comp evaluations, return-to-work clearances, and DOT/fit-for-duty exams
Perform procedures commonly required in urgent care: wound repair/suturing, splinting, I&D, foreign body removal, and basic fracture management
Triage patients, order and interpret diagnostic tests, and develop evidence-based treatment plans
Document patient encounters accurately using the Anthem EMR system
Collaborate with clinic staff and supervising physician to ensure high-quality, compassionate care

Compensation
· Compensation is performance-based, with payment tied directly to exam complexity
· $650 training bonus after completion of onboarding and first finalized exam
Role Overview
· Non-treatment, evaluation-only exams
· No prescribing, procedures, or ongoing patient care
· Flexible PRN scheduling (½ or full days)
· Visits: 30–120 minutes
· Complete standardized DBQs (no narrative reports)
Requirements
· NP or PA with active, unrestricted license
· Collaborative agreement not required
· 2+ years’ clinical experience preferred
Backgrounds: Family Medicine, Internal Medicine, Orthopedics, General Medicine Skills: musculoskeletal, cardiovascular, respiratory, and basic diagnostic exams
Practice Requirements
· Must operate from a dedicated clinical space (no home offices); veterans travel to you
· ADA and OSHA compliant
· Basic setup required (exam room, goniometer, safety equipment)

Qualifications
Active, unrestricted Kentucky Nurse Practitioner license (ANCC, AANP, or AACN Board Certification)
At least 1 year of Nurse Practitioner clinical experience (new graduates not eligible)
Accepted NP specialties: AGNP (Adult Geriatric Primary Care), FNP (Family Nurse Practitioner), ENP (Emergency Nurse Practitioner), ACNP (Acute Care Nurse Practitioner)
Ability to travel independently to patient homes, including rural and underserved Kentucky areas
Strong adaptability, communication, and documentation skills (same-day electronic submission required)
Commitment to patient-centered care and flexibility
Business professional attire or dark-colored scrubs required
Must provide own patient bag with basic medical tools (tablet, PPE, badge, and lab kits provided)
Qualifications
Conduct annual in-home Health Risk Assessments for geriatric patients (1:1 visits, no follow-up)
Perform physical exams, medication reviews, and assess social/environmental factors
Complete basic labs (A1C, fit tests; no blood draws)
Document and submit assessments using Matrix-issued tablet (same-day submission)
Travel daily to member homes within a 60-mile radius (efficient route planning)
Adapt to diverse home environments and patient needs
Maintain high quality scores and compliance with safety protocols
Collaborate with the clinical services team for support and quality assurance
