Your Role:
Provide primary and family care evidenced based care inclusive of accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for our patients.
Conduct assessments, diagnoses, treatment plans, and health counseling
Perform administrative duties including clinic operations and patient follow-up
Deliver virtual care via secure telehealth platform with full EHR documentation
Collaborate with a multidisciplinary care team to ensure continuity of care
Work 30 hours in clinic and 10 hours of telehealth in remote setting.
Required Qualifications:
Active, unrestricted FNP license active state license
Minimum 1 year of NP (2+ years preferred)
BLS certification and ability to pass respirator FIT test
Willingness to obtain multi-state licensure (for telehealth roles)
Proficiency in EHR (EPIC preferred) and strong communication skills
Broadband internet and private, professional remote workspace (for telehealth)
Education:
Master’s Degree from an accredited Family Nurse Practitioner program with current National Board Certification and State of Employment license to practice in the role.
