Position Title: Registered Nurse Practitioner (RNP)
Reports To: Medical Director / Programs Manager
Employment Type: Full-time
Location: Remote / Hybrid

Working Hours: From 8-4
Position Overview:
The Nurse Practitioner (NP) will play a vital role in delivering high-quality, patient-centered care under Chronic Care Management (CCM), Principal Care Management (PCM), and Remote Patient Monitoring (RPM) programs. The NP will assess, plan, and manage care for patients with one or more chronic conditions, coordinate interdisciplinary care, and utilize remote monitoring technology to improve health outcomes and reduce hospitalizations.
Key Responsibilities
Clinical Care & Patient Management
• Conduct comprehensive patient assessments, including medical history, medication review, and risk stratification.
• Develop, implement, and update individualized care plans for patients with chronic and complex conditions.
• Review patients’ clinical charts and sign them, help the billing team to proceed with the billings.
• Provide ongoing chronic disease management (e.g., diabetes, hypertension, COPD, CHF, CKD, etc.).
• Review remote patient monitoring (RPM) data such as vitals, glucose levels, blood pressure, and oxygen saturation.
• Identify abnormal readings or clinical alerts and take appropriate interventions.
• Provide patient education on self-management, lifestyle modifications, and treatment adherence.
Care Coordination
• Collaborate with physicians, care managers, nurses, pharmacists, and other members of the healthcare team.
• Facilitate referrals, follow-up care, and specialty consultations.
• Communicate with patients and families to ensure continuity of care and understanding of care plans.
• Ensure compliance with CMS guidelines for CCM, PCM, and RPM programs.
Documentation & Compliance
• Maintain accurate and timely documentation within the EMR system.
• Record 20+ minutes per month (CCM/PCM) of care management activities in compliance with CMS billing requirements.
• Ensure RPM device setup, patient education, and monitoring compliance meet CMS criteria.
• Adhere to HIPAA and all regulatory standards for patient confidentiality and data security.
Qualifications & Requirements
• Education: Master’s or Doctorate in Nursing Practice (MSN/DNP) from an accredited program.
• Licensure: Active Nurse Practitioner license (State-specific) with prescriptive authority.
• Certification: National certification (AANP/ANCC) in Family Practice, Adult-Gerontology, or relevant specialty.
• Experience:
o Minimum 2–3 years of clinical experience (chronic disease management preferred).
o Prior experience in care management, population health, or telehealth is highly desirable.
• Technical Skills: Proficiency with EMR/EHR systems, telehealth platforms, and RPM technologies.
• Soft Skills: Strong communication, critical thinking, and patient engagement skills.
