Role and responsibilities
Telehealth is a private practice and multi specialty virtual care group focused on providing consistent, high quality follow up care for patients who need ongoing clinical support. We are currently seeking Family Nurse Practitioners interested in a lower intensity telehealth role centered around follow ups, care coordination, and chronic condition management.
This position is ideal for FNPs who prefer a steady, relationship based approach to care rather than high volume acute visits. Most encounters involve reviewing recent visits, monitoring symptoms, adjusting treatment plans, answering patient questions, and helping patients stay on track with their health goals.
The pace is manageable, the workflows are straightforward, and providers have access to clinical support as needed. This role works well for NPs looking for flexible remote work without the pressure of urgent care style visits.
Responsibilities
• Conduct virtual follow up visits and care check ins
• Monitor and manage chronic conditions such as hypertension, diabetes, and asthma
• Review labs, medications, and recent care plans
• Provide education and reassurance to patients
• Coordinate care and recommend next steps when needed
• Document visits clearly and efficiently in EMR
• Communicate with the clinical team for escalations or guidance
Qualifications
• Active NP license in at least one US state
• FNP required
• Comfortable managing chronic conditions and follow up care
• Strong communication and patient engagement skills
• Telehealth experience helpful but not required
• Willingness to obtain additional state licensure is a plus
• New graduates with strong primary care training welcome
