Role and responsibilities
We are seeking an experienced Nurse Practitioner or Physician Assistant to join a well-established, NP and Physician-led internal medicine practice providing in-home primary care and services within independent living settings across the Chicago area.
This role is ideal for a clinician who values autonomy, prefers to manage most patient needs in-house, and is comfortable delivering care in patients’ homes. The practice emphasizes continuity, relationship-based care, and thoughtful clinical decision-making rather than rigid protocols.
Practice Snapshot
Internal Medicine–focused practice
Some wound care and occasional joint injections
Minimal external referrals — most care managed internally
Four dedicated remote support staff who:
Order labs and imaging
Request and track records
Follow up on results
Handle administrative patient communication
Schedule & Workload
Full-time, Monday–Friday
Typical hours: 8:00 AM – 5:00 PM (flexibility with start/stop times as long as work is completed)
Approximately 10 patients a day scheduled in the same area
Driving required within a ~60-mile radius, from South Chicago up to Zion, IL
Compensation & Benefits
Salary: $140,000 starting
Sign-on bonus: $2,000

Scope of Practice
Comprehensive internal medicine and primary care
Chronic disease management
Occasional wound care
Occasional joint injections
Prescribing, including opioid management when clinically appropriate
Collaboration required (support provided)
Ideal Candidate Profile
Comfortable practicing autonomously
Experienced in internal medicine and/or primary care
Confident managing a wide scope of care independently
Comfortable providing care in patients’ homes
Direct communicator with patients and staff
Adaptable, resilient, and solutions-oriented
Comfortable prescribing opioids and setting firm but compassionate boundaries with patients
Requirements
Active Illinois NP or PA license
Eligible and willing to work under collaboration
DEA registration or ability to obtain
Valid driver’s license and reliable transportation
Comfort with in-home care and driving
