Role and responsibilities

We are seeking a compassionate and dedicated Nurse Practitioner to join our healthcare team, working closely with physical therapy, focusing on pain control, and function in skilled nursing facilities.

Key Responsibilities

  • Conduct thorough assessments of patients, identifying and managing medical conditions under the supervision of a Physiatrist.

  • Develop and implement individualized care plans, focusing on function and rehabilitation to improve patients' quality of life.

  • Collaborate with the healthcare team to ensure comprehensive care, including medication management and therapy recommendations.

  • Educate patients and their families on treatment plans, health maintenance, and preventive care.

  • Monitor and evaluate patient progress, adjusting care plans as necessary to achieve optimal outcomes.

Qualifications

  • Licensed Nurse Practitioner with a valid MA certification.

  • Experience working independently while effectively managing your own caseload.

  • Submit accurate and thorough clinical documentation promptly, ensuring compliance with company policies and regulatory requirements.
