We are currently seeking Family Nurse Practitioners interested in a lower intensity telehealth role centered around follow ups, care coordination, and chronic condition management.

This position is ideal for FNPs who prefer a steady, relationship based approach to care rather than high volume acute visits. Most encounters involve reviewing recent visits, monitoring symptoms, adjusting treatment plans, answering patient questions, and helping patients stay on track with their health goals.

The pace is manageable, the workflows are straightforward, and providers have access to clinical support as needed. This role works well for NPs looking for flexible remote work without the pressure of urgent care style visits.

Responsibilities

• Conduct virtual follow up visits and care check ins
• Monitor and manage chronic conditions such as hypertension, diabetes, and asthma

This role is fully remote and gives you the freedom to choose your schedule while seeing a steady mix of acute, chronic, and general primary care telehealth consults.

You will be compensated per visit, and most providers average around 20 virtual consults per day, depending on how many hours they choose to work. The workflow is straightforward, templates are simple, and support is always available when you need it. This is a great fit for NPs who prefer autonomy and a predictable patient flow without the pressure of strict hourly quotas.

Responsibilities

  • Conduct telehealth visits for common acute conditions and episodic care

  • Manage routine primary care follow ups and chronic condition check ins

  • Review labs and diagnostics when available

  • Develop treatment plans and prescribe as appropriate

  • Complete documentation within the platform’s EMR

  • Communicate with the support team as needed for escalations

Qualifications

  • Active NP license in at least one of the following states: TX, FL, CA, GA, MA, NY, NC, AZ

🎓 Qualifications

  • Active, unrestricted, and unencumbered Nurse Practitioner license in at least one U.S. state.*Multiple state licenses are highly preferred.

  • Prescriptive authority as a Nurse Practitioner.

  • Active national board certification (FNP, WHNP, AGNP, or similar).

  • Minimum 3 years of recent experience (within the last 5 years) practicing as a Nurse Practitioner in Primary Care, Women’s Health, or Gynecology.

  • Ability to work independently and make sound clinical decisions.

  • High proficiency and efficiency with technology (telehealth platforms, EMRs, communication tools).

  • A strong passion for caring for women navigating menopause and midlife health transitions.
