Permanent openings due to organizational growth
Seeking a board certified or truly board eligible nurse practitioner or physician assistant
Must have at least two years of post training primary care experience
Opportunities available in Dorchester, Quincy, Lynn, Brockton, and Beverly
You would work at one site only
Medicare-focused patient population (65+)
Hybrid schedule
Four onsite days per week from 8:00 AM–5:00 PM
One remote day per week
Monday-Friday schedule
Part-time option available
Light call schedule
Approximately two evenings per month
Approximately two weekends per year
Average patient volume: 15-18 patients per day
Comprehensive, team-based primary care model
Focus on chronic disease management, preventive care, and social support
In-clinic and virtual care options
Coordinate care with specialists and hospitals
Emphasis on social determinants of health including housing, food security, and loneliness
Work alongside physicians and APPs
EMR: Athena
